Sample Memo: Requesting PDAS Every 5 Years
The following wording may be used to
request a formal PDAS appraisal only once every five years. This sample document
includes references to the state appraisal rules. Please do not print this page and
submit it to your appraiser. You should create a document of your own, inserting the
appropriate information where indicated. Call the ATPE Member Legal Services

Request for PDAS Appraisal Every Five (5) Years

This memo is written in accordance with
Sections 21.203(a) and 21.352(c) of the Texas Education Code.
State law now allows districts to appraise proficient teachers
less frequently but at least once each five years. On my last
formal PDAS observation, from the school year 20__ - 20__, I
received scores of proficient and/or above, and I did not have any
areas rated below expectations or unsatisfactory. (Please see copy
of last PDAS observation attached.)
As I meet the requirements outlined above, I agree in writing and
respectfully request that I receive a formal PDAS observation only
once every five (5) years, as opposed to annually. I would
appreciate your letting me know your decision. I would also be
happy to meet with you to discuss this matter at your convenience.
